We’ve designed a way to get you the help you need at every step of your recovery following your hospital stay. To make accessing help as convenient as possible, we’ve designed the services via a Tele-health model. You’ll be able to connect to your Care Coordinator via video on your phone. Your manager will make sure that you receive:
Appointments coordinated with specialists and your primary care doctor
Medications ordered and coordinated with your current medications
Any support you need to access transportation to doctors’ visits, food, and durable medical equipment
Support in the language most comfortable for you
24 hour support
Direct coordination with your primary care physician
Our program, which focuses on making sure you are closely followed by expert Nurse Practitioners and your Primary Care Physician following your return home, has been proven to dramatically reduce the likelihood that you need to return to the hospital.
